by writing to the floppy drive on my machine. My floppy driver is compiled as a module and autoloaded by kerneld. (It doesn't matter whether or not the module is loaded permenantly by modprobe or autoloaded. I haven't tried it compiled in) My ethernet card is an AT-1500BT with the Lance chipset.
I generated this particular mess by first running: dd if=/dev/fd0 of=floppy which did not produce any errors, and then dd if=floppy of=/dev/fd0 which generated all of the above messages. The errors only occur when the floppy is actually being written to physically (light is on). Playing around in the cache does not cause any problems.
My kernel is: Linux version 1.3.94 (root@vorlon) (gcc version 2.7.2) #1 Mon Apr 22 16:53:48 CDT 1996
My machine is a Gateway DX-100 VLB (Micronics motherboard ~1993) with on board IDE and floppy controller - floppies connected here AT-1500BT Adaptec 2842A with floppy interface disabled PAS 16 with SCSI disabled 24 Meg memory Diamond Viper video card
I'm willing to try other kernels etc if someone requests it.
