I think Don tried to config ide-scsi without configuring scsi.
The config file appended to Don's mail is the one which worked
(after he has added scsi configuration).

Don, Do you really need ide-scsi emulation? In that case the
scsi driver will take over your ATAPI devices (eg, cdrom).
So you need to configure scsi cdrom but not atapi cdrom.
Unless if you really need ide-scsi (eg. if you have an atapi
cd-writer) I recommend that you turn off ide-scsi emulation (in
the block-device menu) and scsi (in the scsi menu).
