Subject[PATCH] x86: fold _PAGE_GLOBAL into __PAGE_KERNEL
With the iounmap problem resolved, it should be OK to always set
_PAGE_GLOBAL in __PAGE_KERNEL*.

[ Did this patch cause problems before? ]
