I know you have looked into this a bit in the past.
Time and effort are the main issues I know of. First we'd need some kind of FD-per-endpoint infrastructure, then it should be easy to implement the AIO file ops, one kiocb per URB ... much like "gadgetfs" does for USB peripherals, one kiocb per usb_request.
