Subject[BK PATCH 2.5.6-pre2] Upport SCSI reset/reservation fixes from 2.4.19-pre2

The attached represents a straight port of the reset/reservation handling 
fixes that went into 2.4.19-pre2, except for the removal of calls into the old
error handler.
