Subject[PATCH] omnibus NON-include/ cleanup (less big)
David Weinehall thought my previous posting was useful,
so this is the other half. This is a followup to

which patched up #defines under linux/include/.

Attached below is a patch to fix the rest of the headers not under
linux/include/. It applies to linux-2.4.15-pre4.
