just use any old vga, with a bit of level-shifting circuitry...

there are always a few overpriced cards around that do it: based on market research that asks do people really want vga output on tv? instead of do people really want vga output on tv, even if it looks so rough?
the limitation is the monitor quality, not the card, so the market is always disappointed. hence you often see them cheap, between droughts.
if you can't find a vga-->tv card cheap ($10), configure a normal vga as per the modeline quoted in lg tip above, and wire up a little sync-juggler, feeding into a monitor-with-separate-sync, or a $5 video modulator (little box you'll find in every video game, amiga, vcr, etc)
