This is the second pass of removing superfluous "__KERNEL__" preprocessor tests from headers under include/linux that aren't exported to userspace. The first pass (the "no-brainer" pass) dealt with headers whose *entire* contents were wrapped in a __KERNEL__ test, while this pass goes on to handle those headers that had at least a *little* content outside of the __KERNEL__ tests. (Thus, there is no overlap between the patches.)
This pass doesn't deal with any of the ext*.h or jbd*.h headers, since those headers had a little too much __KERNEL__ testing for my comfort level.
