I just saw my very first instance of this problem. It occured during a torture test where I try to tar the entire filesystem to /dev/null from a remote NFS client:

Kernel 2.2.13 + valinux nfs patches
I'm quite sure there are no hardware problems with this box. It's been utterly stable otherwise.
