Matthew, would you mind testing the current pre-112? The problems you see may be due to the IO-APIC code, but before I start blaming that let's fix the other things to make sure it's really the IO_APIC changes.
This essentially disables the "clever" (or not-so-clever) SMP wakeup function. Especially during bootup we can be woken up even before SMP has even been initialized fully. It also switches the bug finding around again.
So, does it boot again with pre-112-1, and if it does, is it stable for you?
