Some bug plonks the X server hard, the user (jumps to another VT|logs in from another box) and kill -15 X, no result, same user now uses kill -9, graphics card jams.
This is a very likely scenario given the size of the X executable (the bug can easily be in a dynamically loaded module like GLX).
