Just for the record, I had a bit similar problem with our production server, which is running 2.1.76 and has HP 6020 SCSI CD-R. It didn't report errors or anything like that, but after about the 30th minute there were stops in the audio, like short rewinds etc. Practically all tracks after 30min were unusable.
I upgraded cdda2wav to 0.95alpha13 and now it reads CDs 100% fine. It shouts all kind of SCSI-errors, though, but they do not seem to matter anything. Perhaps it works now because there's -speed option in this new version and we're using that to set reading speed to 2 or then something else simply has changed in the cdda2wav. I can't remember what version the older cdda2wav was.
Why that effect started to show after 30mins, I don't know.
