I have verifyed that the "swap_duplicate() unused entry" messages while swapoff -a is running, are perfectly safe. What happens is that while read_swap_cache_async is sleeping on the __get_free_pages(), another process swapin the page from the swap and then swap_duplicate() returns 0 and everything then goes fine.
