Did you read ftp://ftp.suse.com/pub/isdn4linux/README ?

In short: 
    unpack the tar file for example in /tmp/i4l
    cd /tmp/il4
    ./std2kern -d

    cd /usr/src/linux
    make config 
etc.

Make sure to get the right version for 2.1 (not 2.0) !
